Cardboard sleeve. I bought it a few years ago. It was limited to 5000 copies and came with Formulas Fatal To The Flesh a few years ago. It also had a signed guitar pick from Trey in it.
It's cool to have but I can't say i listen to it much,it's just the guitar solos from Domination and Formulas.
