Definitely old Roadrunner. Unique Leader had some okay releases, but fuck them. The label that I've been most impressed by over the last few years is Willowtip. Consistent in releasing great albums.
Neurotic is pretty new, as in not even a year old, but Ruud will have a great label on his hands in a few years.

hey man, I just heard some willow tip bands at friends from a torrent pack and holy shit they were awesome!
that label has some pretty well known and hard hitting bands but the smaller ones were great as well

When you really think about it, they were the first big metal label, and others like century media wouldn't really exist if they hadnt been there in the start, but then again, there would be no need for labels like century media if roadrunner didn't go shit.
