i use the .60 dunlop nylons or the .60 dunlop ultex. nylons for thrash, ultex for leads and slower stuff. (they dont get used for slow very much)

I use dunlop 1,5 mm or 2,0 mm. I also use fender medium or extra heavy picks. I play death/melodic/heavy metal and i play lead.

dunlop black and red jazz III or dunlop gator grip 2mm for anything electric, fender mediums or funlop .75-1mm for acoustic stuff
